Hydrocele

A hydrocele is a fluid-filled sac surrounding the testicle, leading to swelling in the scrotum. It is usually painless and commonly occurs in newborns or older men due to injury, infection, or inflammation. While hydroceles are generally harmless, large or symptomatic cases may require medical intervention.

Causes of Hydrocele

  • Congenital Hydrocele – Present at birth due to incomplete closure of the tunica vaginalis.
  • Injury or Trauma – Scrotal injury can cause fluid accumulation.
  • Infections – Conditions like epididymitis or sexually transmitted infections (STIs) can lead to hydrocele formation.
  • Inflammation – Testicular or scrotal inflammation can result in excess fluid production.
  • Post-Surgical Complications – After hernia repair or other scrotal surgeries.
  • Aging – Fluid imbalance in older men can contribute to hydrocele development.

Symptoms of Hydrocele

  • Swelling in the scrotum, which may be painless or cause mild discomfort.
  • Heaviness in the scrotal area, especially after prolonged standing.
  • Fluctuating size, with the swelling increasing during the day.
  • Possible pain or discomfort if the hydrocele becomes large or infected.

Diagnosis of Hydrocele

  • Physical Examination – Doctor checks for scrotal swelling and firmness.
  • Transillumination Test – A flashlight is shined through the scrotum; clear fluid confirms a hydrocele.
  • Ultrasound – Used to differentiate hydrocele from other conditions like hernias or tumors.
  • Urine & Blood Tests – To check for infections if suspected.

Treatment for Hydrocele

🔹 Observation – Small, painless hydroceles in infants often resolve on their own.
🔹 Aspiration & Sclerotherapy – Fluid is drained using a needle, and a sclerosant is injected to prevent recurrence (less commonly used due to recurrence risk).
🔹 Hydrocelectomy (Surgical Removal) – Recommended for large or persistent hydroceles. A small incision is made to remove the fluid-filled sac.
🔹 Laparoscopic Hydrocele Repair – Minimally invasive option for hydroceles associated with hernias.
🔹 Antibiotics – If the hydrocele is caused by an infection.

Post-Surgical Care & Recovery

  • Wear supportive underwear to reduce swelling.
  • Apply ice packs for the first 48 hours to minimize discomfort.
  • Avoid heavy lifting and strenuous activities for 2-4 weeks.
  • Keep the incision site clean to prevent infections.
  • Take prescribed pain relievers if needed.

Prevention of Hydrocele

  • Protect the scrotum from injury by wearing proper sports gear.
  • Seek timely treatment for infections to prevent fluid accumulation.
  • Monitor scrotal swelling and consult a doctor for any unusual changes.

Yes, in infants, hydroceles often resolve within 6 to 12 months, but in adults, persistent hydroceles may require treatment.

Generally, a hydrocele does not affect fertility, but if it coexists with conditions like varicocele or infections, fertility might be impacted.

The procedure is performed under anesthesia, so there is no pain during surgery. Postoperative pain is mild and manageable with medication.
